Permissions reset after a macOS update
A macOS update or a reinstall of Bouncer can drop the Accessibility and Automation grants. Bouncer checks the real state each time it launches, so Settings > Browsers shows what is missing.
Open Settings > Browsers and select Safari, Arc, or Dia. A banner at the top names the missing permission. If both grants are gone, the two-step setup view appears instead.

The default browser can reset at the same time. Click the Bouncer icon in the menu bar and look for Set as Default Browser…. See Links aren’t routing at all.
